Step 1. Obtain A Copy Of Your Medical Record Indicating Your Qualifying Condition
You are qualified to apply for a medical marijuana card in New York if you have been diagnosed with one of the specified medical conditions.

As supporting documentation, it is necessary that you obtain a copy of your medical records indicating the qualifying diagnosis. The following are lists of qualifying conditions that make you eligible for a medical marijuana card:

Step 2. Complete A Certification Visit
After submitting your records, the next step in the process is getting certified. Your certification will be issued by our practitioners who are registered with the New York State Medical Marijuana Program.
